  • Urinary Catheter Care following maintenance bundle
  • Observe/Record Intake
  • Observe/Record Output: 1. Urine, 2. Stool, 3. Emesis, 4. Nasogastric, 5. Jackson Pratt Drain
  • Performs EKG
  • Telemetry lead application and maintenance
  • Care of the patient being admitted
  • Care of the patient being transferred/transported
  • Care of the patient being discharged
  • Care of the Patient with Diabetes/Hyperglycemia
  • Discusses additional care needs of the patients: 1.Suspected Abuse or Neglect, 2. End of Life, 3. Bariatic patient, 4. at Risk of Harming Self or Others (Patient Safely Companion Role)
  • Suction and Oxygen Equipment: 1. Wall regulators, 2. Reviews continuous vs intermittent suction, 3. Portable equipment for transporting patient, 4. Sets up/changes tubing and canister
  • Stores equipment in designated location
  • Cleans equipment after use with each patient with proper cleaning solution and dry time
• Patient Mobilization/Transfer
  • Beds: 1. 2 side rails are up, 2. Bed in low position, 3. Bed locked, 4. Zeros bed, 5. Sets bed exit alarm, 6. Sets iBed
  • Utilizes proper fall prevention interventions
  • Bed to Chair
  • Chair to Standing
  • Assist with Ambulation
  • Assists with Range of Motion
  • Assistive Devices: a. walker, b. cane
  • Positioning
  • Lift and friction reducing devices
• Infection Control
  • Hand Hygiene
  • Proper Bag Technique
  • Personal Protective Equipment
  • Follows Standard Precautions
  • Follows Isolation Precautions (i.e. contact precautions, c-diff precautions, droplet precautions, etc)
  • Equipment Care (allows proper drying time for cleaning product)
  • Follows bundles to prevent hospital acquired conditions (CAUTI, VTE, Pressure Ulcers, Surgical Site Infection)
• Standard Procedures
  • Verbalize an understanding of patient centered care
  • Uses AIDET, hourly rounding and communicates appropriately with patient and family
  • Demonstrate individualized approaches to client care in verbal and physical interactions with clients

  • Reviews Electronic Documentation Requirements: a. legible, b. timely, c. accurate, d. complete, e. relates to Plan of Care, f. communicates Abnormals to RN
